Your First Hearing Evaluation
At your appointment, we will take your case history, perform an audiological evaluation and have a discussion of the results. Most of our patients find it helpful to bring a spouse, friend or family member to this appointment. We encourage you to do so. If hearing devices are an appropriate treatment, you will have the opportunity to be educated on the wide variety of options available.
The hearing evaluation will be billed to your health insurance. You will be responsible for any co-pays or deductibles not covered by your health insurance.
Note to all Medicare Patients
You will need to bring a prescription for audiologic evaluation and management from your family doctor. If you do not bring a script, you will be responsible for all charges incurred at your appointment. If you have any questions regarding obtaining a script, please call our office at 850-298-8407
